my 04 extra cab has a 2.7l while idling i have a whining noise almost like a power steering whine.. but i have a brand new rack w power steering pump... i check the idler pulleys for both the power steering and ac compressor they are not the culprit either i took each belt one at a time an tested while idling alternator is good ac compressor is good and power steering my only thought could be the crank... has anyone had this before ... i was considering a new harmonic balancer if anyone has gotten in to this any advice and wisdom would be awsome :):):):):)

water pump maybe? Mine does this too but only on the first start of the day. Only whines for maybe 5 seconds then goes away. My issue though I know is the water pump...had to replace probably 5k ago.

    Only cosmetic. (For now)
I have the same issue in my 99' 3.4. I replaced the water pump and timing belt myself and it began making that whine as soon as the new parts were in it. Everything was replaced and I torqued all the bolts to spec. So far I have almost put 5000miles on it and nothing has changed in terms of performance/fuel economy, just whines in the lower RPM/idle range. Zer0 Mine whines like that on coldstart up too, but its a much different whine as you said and lasts for 5 seconds ish. My taco has to start in -5 fahrenheit (-20C) daily and it makes that noise when it is really cold. But that other whine is always there no matter what engine temp. If anyone know's what this phantom whine is let me know too, I'm almost tempted to go back in there and check everything to set my mind at ease.

It's less Tacoma and more mod
Any chance you drive a manual? In neutral the input shaft (main shaft) is spinning a rack of gears and can whine if the tranny bearings are wearing out or if you have gears that are under lubricated. Mine make a little whirring noise when in neutral with foot off the clutch.

It is a manual tranny but is makes noise when it's in gear or in neutral I'm a 143k miles the engine runs great only thing I need to do is replace rear main seal there is a tiny bit of oil seaping out the bottom of my bell housing it doesn't burn oil ... The clutch fan doesn't have any play in it ... I'm thinking it has to be the crank pulleys or harmonic balancer

That seems like a pretty solid test for the pulleys to me... when did the noise start? recently or has it always been there?

Edit: I'm thinking maybe the timing chain is getting loose considering your mileage... possibly rubbing on some components under the timing cover, or on the timing cover itself causing the noise.

It has done it for at least 4000 miles it gets a little louder and more noticeable at higher Rpm I know the timing chain hasn't been replaced Ive got time to get into it this weekend and the weather isn't as shitty I'll try to take some pics and post them

see if you can wiggle the outer pulley on the balancer. I just had to replace my balancer because the outer ring where the belt rides seperated from the rubber and would spin on the rubber.

If it is the input shaft bearing it WILL make the noise in or out of gear. Does it still make noise when you depress and hold down the clutch pedal?

It was a combination of water pump, the two idler pullys and alternator ..... Now it runs a lot better and quiet as a mouse
