Truck jerks when i come to a stop

OK, so heres my problem. When I am driving and I slow my truck down to a stop, If I slowly release the brakes my truck kind of jumps ( feels like its coming from the hood.) Should I be worried? Also, how would I fix that. Thanks!!!

 

I'm just throwing this out there and I'm probably wrong but are your parking brake cables ok and releasing the brakes or broken/seized and stuck on?

 

Sounds like axle wrap. Basically the leaf springs are worn out and when coming to a stop it allows the axle to rotate a bit. When you release the brakes you're releasing the tension hence the "jerk" feeling. It can happen upon slowing down as well as acceleration.
It can be fixed with new leaf springs, or an add a leaf to add a little strength back into them.

Are your leafs pretty much flat, small arch up like a smile, or do they have a frown shape?

I bought my 99 new with an A/T and had that issue from the beginning. When I came to a stop and let off the brakes the back end would sit down and clunk like axle wrap that others mentioned in this thread. I took it to dealer and they couldn't find anything wrong. Years later I changed all of the vacuum hoses and it went away! I believe one of the hoses going to the 2 transmission solenoids had a sharp bend and was choking off vacuum so it wasn't down shifting properly. It has been this way since the factory. These are the 2 solenoids on the left side of engine compartment behind the battery. Also, changing all of the vacuum hoses made a big difference in performance. It's like doing a tune-up.
