    My 2009 with a 2.7/5 speed and very high mileage is really being sluggish in 2nd gear until i get it to 3000 plus rpm, what could it be?

    Sluggish and slipping are quite different. A better explanation of what it’s doing would be great.

    I rev high on my truck. Usually shift around 3500-3700 rpms or higher. There is a dead zone around 2500 rpms when the truck just doesn't want to accelerate.
    Don't be afraid to push the little 2.7- it can take it!!

    Is this a new problem?

    What is "very high mileage?"

    Sluggish from idle to 3K ONLY in second, or in higher gears (especially 3rd) as well?

    Would be interesting to look at any stored codes, even if there's no CEL.

    I have my suspicions, but I don't want to speculate w/o more data.

    Just happening in second gear, it makes the truck hop. Have shifted to second without using the clutch and same problem

    Id start by cleaning the MAF sensor and throttle body. Sounds like its stumbling and not the clutch slipping. To test the clutch, while driving around 40 mph, shift into 5th gear and floor it. If the motor lugs down, the clutch is still good. If the revs shoot up and you dont feel any forward movement, then the clutch is slipping

    The odd part to me is, that you are not able to replicate the issue in any other gear.
    I would think, if it were strictly an engine problem, you could get the issue to occur in other gears.
    Engine load can differ in each gear. Though typically most manufacturers try to keep the load/torque flat across the gears.

    Have you short shifted or held the shifts higher to see if you can get it occur in other gears?

    I think in my older PreRunner 2.7 it seemed to be the widest ratio between the gears?
    I honestly can’t remember?

    Manual transmissions don't "slip." Highly worn clutches slip, and worn bands in automatic transmissions may slip, but manual transmissions are unable to "slip."

    Sounds like clutch slipping. I've have typically noticed it beginning in 2nd gear. Torque is not uniform per gear, else acceleration would be consistent from gear to gear.

    I got a lot of black gunk out of the TB, have not messed with the MAF yet. Still the same, did punch it at 40 mph in 5th and get worked fine

    If the clutch didnt slip and it wasn't the TB, it could just be a dirty MAF. Make sure you only use MAF cleaner on it as it is a senstive electronic piece. If the wire in the MAF is dirty or gummed up, it could be causing your issues. Make sure not to touch the wire on the MAF while cleaning it. Just spray, let dry, and repeat
