My '04 Tacoma is Still within (barely) and eligible for the 15 year DOS extended frame perforation warranty so I decided to take a good look at the frame one last time and found this: The front left cab support has a greater than 10mm perforation as circled in the photo.
Is this enough for Toyota to replace the frame? The truck goes in next week for state inspection (PA) and I'll bring this up to the service writer.

I am not sure if the cab mount is considered part of the frame.

I bet with some serious looking I could find enough

Well, it's welded on and cannot be purchased separately. Looking at the frame in the parts catalog it clearly shows the mount as part of the frame. The rest of the frame seems solid.
The coating Toyota applied blocked the drain holes where the mount is attached to the frame.
I have all the paperwork and will see next week what the dealer has to say about it.
PA state inspection clearly states that the frame must be solid to pass.

Fyi, the right rear body mount (like yours above) is what failed my frame, to get a brand new one!! Literally, mine looked the exact same as yours (condition wise) and the Toyota dealer failed it. They said the rest of the frame was good, except that body mount. Needless to say, fast forward 4-5 months, I had the new frame, new brake lines, gas lines, body mounts, transmission mount, motor mounts, lower control arms, lower ball joints, gas tank skid plate, gas tank straps, and new rear leaf springs. Total bill that was sent to Toyota Corp, was $17,600!! I hope you luck out like I did, and get the new frame. It is so awesome once your truck has the new frame!!

WOW! They really treated you right. I got the new frame, but not the gas tank (even though they tried to sell me one because they said the orig was leaking).....Still happy w/ new frame though!

Also, just a fyi. I had my frame replaced last year, not this year. As my truck is a 03. I brought it in to Toyota dealer 5 days before the 15 year frame warrenty ran out!!!

Go over the entire frame with a hammer, you should be able to find more, the mount failure might be enough but I would find more to strengthen your case. Good luck!

After they found the hole in the cab mount. I spent two hours under there peeling off the coating where it was rusted underneath. I ended up with 1/2 a 5 gallon bucket of rust flakes and undercoating but surprisingly the rest of the frame appeared solid.

Took my truck to the dealer today and a new frame is on order! 4 weeks before the extended warranty expires. Probably one of the last ones done under the 1st Gen extended campaign.

I just went though this. I recently bought my '04 (July), and I specifically asked my local dealer (3x) this summer if the truck had been serviced under the frame campaign, and they would not tell me (something about it being private information). Only later when I called Toyota directly in early September did they confirm the frame was treated and had the extended warranty, which of course, expired 10 days prior. My frame looks ok, but I found a rather nasty hole, which precipitated my call to Toyota directly. If the dealership had given me info in a timely manner, I could have, at least, gotten it evaluated. Now Toyota won't even consider it.
Some 2004s may still qualify if they had a late 2004 in service date, but you need do some detective work if you are not the original owner.
And, if you're in Western NY, don't count on Northtown Toyota to help you at all.

I hope you documented this. If you brought a problem to them before the warranty expired and they didn't fix it they are legally required to make good.

Yes, I did. I opened two cases with Toyota (one for the frame issue, one for the dealership). However, since the dealership did not inspect the vehicle prior to the warranty expiration, I'm SOL. The key event was when I went into the dealership this summer. I inquired about the frame status, but the said they had no records relating to it. I just got off the phone with Toyota, and they confirmed that the dealer could have informed me at that time that the truck had an extended warranty, or alternatively told me that i should bring the truck in for inspection; they did neither. I made an assumption that the frame was ok, because, for the most part, it does look ok. The dealership eventually did an inspection, and documented the rust hole I found, but this was after the fact.
Again, the key action to take is to have the dealership inspect the frame. They must document it to Toyota before the warranty expiration date.

It sounds like your frame is in pretty good shape, it sucks that you missed out on the warranty, I feel the same about my frame but my plan now is to move forward and have a good plan to protect the frame I have. I plan to grind off any weak spots ( I saw to small spots) and weld it up, then use a 360 nozzle with and air gun to really hose down the internal frame with preventative.

Go talk to a lawyer. If you brought the complaint to their attention before the warranty expired, they are legally bound to make good on the complaint.
