Just a quick background. It's on 2007 Double cab 4WD with almost 200K miles. I had the truck in for an alignment about a months ago, however when got it back the steering wheel was sitting at the 11 o'clock position and doesn't pull. I took it back and they align it again, and this time it's sits at the 12 o'clock with no pull. Now, I noticed the steering is back at the 11 o'clock again with no pull. I know I didn't hit any curb, pot holes or anything like that. I noticed about a month ago my steering fluid was low so I filled it to the correct level, but there's no noticeable leak. I do noticed a rumble around 60-70MPH only when I have the accelerator press and goes away when I let go. There's new tire that's been road force balance, new hub/wheel bearing, struts, brakes and new driver side CV axle, and tie rod ends are in good condition. On the highway the steering feels loose like it's jumping from the grove in the road side to side. So I'm thinking there maybe and issue with the steering rack? Could it also be this is due to the the fact that I just had my leaf springs replaced by the dealer due to the recall? Since the recall was done yesterday and I noticed the steering wheel off centered today. Thoughts?

The rear leafs won't affect the steering wheel.
Is the steering linkage good and tight?

Yep, check the linkage that attached to the rack and also checked steering colum under the dash. They both seemed tight

What about the steering wheel itself?

Seems to turn fine, no binding, no pull to either side, no strange noise. Just the steering wheel is not stright.

My lower control arm bolts came loose a while back and it caused my steering wheel to be off while driving down the road. I'd have them check the alignment again.
THEN, when you get it home, put it up on some ramps (weight of the vehicle fully on the tires) re-tighten that hardware to (I think) 100 ft*lbs. You'll have to remove your front skid plate, but if it feels like the steering wheel turns without binding or feeling strange, I'd guess you slipped back out of alignment.
IF that's the case, don't wait too long, you'll scrub a lot of tread off your tires....ask me how I know.
I also had one alignment tab that was bent flat.

His cam bolts might be frozen in place with the age of the truck

Actually I loosen the cam bolt myself when I replaced the struts. I used the no spring compressor method.

I didn't think it would be that easy and so quickly, especially how recent I got the alignment done twice. The low steering fluid was what had me assume a bad steering rack.

Worn ball joints will do this, it will sit right at some angles and bad at others.

Possible tie rod wear is likely as well. The ujoint on the steering shaft could have play too.

I just took the truck back out and drove it on the highway. Used all 3 lanes, now there is a very slight pull to the left that's most noticeable 65+ MPH. The pull doesn't happen speeds less than that. If I center the steering wheel the truck will go to the right. At 55+ There is an out of balance / rumble sound, and vibration on my feet. The sound is similar to wind noise from having your back your window row down while driving down the road. There's no shake coming from the steering wheel. I'm still thinking it might be a rack, inner or outer tire rod, ball joint issue. It's hard to understand how it can come out of alignment that quickly just from normal driving in the highway or city.

Sounds to me like your cam tabs are bent and the control arms are moving with road pitch change. Have you shaken the truck down already? Is the tire solid? Bad control arm bushings or LBJ’s?
